Chestermere Postpartum

Maternal/primary caregiver and infant assessments and screenings for families following discharge from hospital, and for first 2 months as required following birth.

This service provides care to families who have an infant 2 months old or younger. Care includes physical assessments of the birth parent and baby, psychosocial assessments, jaundice screening, Newborn Metabolic Screening, infant feeding support,guidance and teaching on a broad range of topics regarding infant and maternal care, and referrals to other AHS departments or community referrals as necessary.

Eligibility

Must have just delivered a baby within past 2 months. Service is for the birth parent and the baby.

Service Access

Referral is direct from Acute Care, following discharge from hospital. Families who move to the area with a baby under 2 months of age can obtain an appointment with PPCS by contacting Health Link or through a physician.

 

Initial referral is from Acute Care upon discharge or through Health Link.
