People who need IV therapy for longer than 3 days and who meet the conditions to have it at home rather than in the hospital

Connect Care Users: Use "Internal Referral (to Connect Care Department)" on the Ambulatory Referral Order in Connect Care

Non-Connect Care Users: Please send referral letter with required information to our clinic.

Referral letters must include:

•  diagnosis
• antibiotic therapy including medication name, dose, frequency and stop date
• any weekly blood work
• date of patient's follow up appointment

Cost of medication is billed to the client. Some or all of the medication costs may be covered by any existing drug coverage that the client has.
